Couch & Associates Customer Satisfaction Applet

Couch and Associates is a web services company who wanted an interactive customer satisfaction applet and they came to us to get it done fast. Using Couch's logo and gray/yellow/black corporate colour scheme, we designed and built this interactive questionnaire which communicates the information gathered directly to Couch's servers on completion.
